Job Description

This clinical position reports to the Clinical Medical Director, and will play a critical role in making a meaningful difference in the lives of this vulnerable population, which suffers from the highest levels of obesity and diabetes in the nation.​ ​

Essential Job Duties & Responsibilities:

  • ​​​​Provides compassionate and high-quality patient care and implementation of evidence based medical practices.
  • ​​Serves within a multidisciplinary clinical environment to strategically improve health outcomes for empaneled patients in a coordinated manner.
  • ​​Collaborate closely with a team: physician and health coaches (advocates that will help patients set wellness goals, drive activation, and improve self-efficacy)
  • ​​Support onsite and community-based efforts that aim to drive patient engagement, access, population-based patient management, referral tracking, mobile clinic, and clinical quality performance evaluation.
  • ​​Perform other duties as assigned

Qualifications

  • Graduate of an accredited NP or PA program with national certification.
  • ​​Minimum 2-5 years of clinical experience, preferably primary and urgent care.
  • ​​Valid DEA number.
  • ​​Weekly travel between Delano and Lost Hills locations
  • ​​Proficient with technology, including MS office, tablets, smartphones, and EMR systems.
  • ​Strong interest in driving population health.
  • ​​Written and verbal fluency in Spanish preferred.
  • ​​Travel required to Lost Hills Clinic, Delano clinic, and Mobile clinic.​ ​

Knowledge, Skills & Abilities:

  • ​Strong background in primary care.
  • Management of long-term diabetic patients desirable.
  • Provide quality care in different environments including and up to working in a mobile clinic at our outlying plants.
  • Strong team player and ability to work independently. ​

Wonderful's dedication to you:

  • Pay Range: $150,000.00 - $175,000.00 and is eligible for a discretionary bonus. Final compensation will be dependent upon skills & experience.
