The RN is responsible for delivery of patient care to individuals, and families in an ambulatory care setting. In partnership and collaboration with other health care professionals, the RN addresses patients’ wellness, acute illness, chronic disease, disability and end of life needs while maintaining the standards of professional nursing practice. The RN team leader is expected to demonstrate leadership skills within Crystal Run Healthcare, and in the community.

Primary Responsibilities:

  • Prioritizes activities and staffing needs based on the patients’ immediate health need or the nurse’s judgment of anticipated patient needs
  • Documents and communicates the information and data collected in a retrievable, understandable, and readable format
  • Gathers input form staff, patients, and families on ways to improve patient care and departmental services
  • The RN team leader plans and identifies strategies to attain expected patient outcomes
  • Facilitates problem solving and continuous quality improvements through participation in ongoing department projects, staff huddles and participation in department meetings
  • Assists the staff in developing and maintaining competency
  • Completes staff schedules and assignments in a timely manner based on appropriate employee skill level and available skill mix
  • Implements the plan of care in partnership with the patient, caregivers, and other healthcare professionals in a timely manner
  • Performs interventions supported by evidence- based ambulatory care nursing practice during clinical visits, telephone encounters and electronic communications according to regulations, standards and organizational policy and procedure
  • Evaluates staff and reports findings to nursing leaders, to ensure assessment and data gathering practices are timely, reliable valid and comprehensive according to practice standards
  • Collaborates with nursing staff using shared decision making, to implement the scope and standards of practice within the department
  • Partners collaboratively with others at all levels of the organization to create an environment in line with the Core Values of Crystal Run Healthcare
  • Fosters communication and coordination of workflow improvement efforts

Required Qualifications:
  • Graduate of an accredited School of Nursing
  • Possess a current NYS License as a Registered Professional Nurse
  • Specialty specific certifications as required
Physical Requirements:
  • Work is active, predominantly standing and walking
  • May occasionally be required to assist with lifting people

New York Residents Only: The hourly range for New York residents is $33.75 to $66.25 per hour.
